Patient Forms

Upon entering our office our receptionist will greet you and welcome you as a member of our chiropractic family.  We will request you fill out our New Patient Forms provided by the receptionist or the website.  The paperwork provides us with general information about yourself and your condition.


Next you will have a consultation with one of our staff to discuss your health-related problems, concerns, as well as potential treatment options. Your first visit is designed by the doctor to learn more about you, your condition and your expectations to determine whether chiropractic care will you meet your goals.


After your consultation, the doctor will perform a complete chiropractic examination testing your reflexes, your ability to turn and bend as well as other standard neurologic, orthopedic, postural, and physical examinations.  The doctor will gain valuable information regarding your condition so that you have a better understanding of how chiropractic can help you. If necessary, the doctor may take X-rays or refer you for additional testing procedures. Nothing will be done in our office without your consent.

X-Ray Studies

Your specific condition may require us to take x-rays to get a full perspective of your spinal health. We do not x-ray every patient that comes into our clinic as a new patient.  On average, we take x-rays on a third to a half of our new patients.  X-rays help us rule out more serious conditions or assist us in developing the most effective care plans for you.  Most people are amazed once they see their x-rays and can often immediately identify their misalignments and degeneration themselves.

Report of Findings

Once all the information and examinations have been performed, the doctor will give you a report of findings to answer the three most popular questions: 

  1. Can you help me?
  2. What do you recommend that I do to get better?
  3. How much is it going to cost?

After discussing with you your history and your goals, examining your spine and reviewing any x-rays or tests, the doctor will discuss recommendations with you. If you have a condition that requires care with other providers, the doctor will tell you. If the doctor believes we can help you, we will recommend treatment and a wellness program.

Chiropractic Adjustments


Chiropractic adjustments restore normal motion the joints of the spine and improve the function of the nervous system.  Most of our patients begin to see results from the very first adjustment. Our gentle yet effective adjustments, are performed with a hand held instrument.  We are able to adjust infants to the elderly with great success.

Hurst Chiropractic has such an awesome staff! They have always been so good to me! Dr. Chad has been so helpful with the adjustments and even explaining stretches to help.
